Required WCL 22 and WCL 26 forms to be completed for a patient that the Doctor treated, however, the rude receptionist was unable to assist because she cant print or scan (in 2020, imagine) and requires the patient to physically drop off forms, thereafter, make an additional trip back to the surgery to collect the completed forms.
